Terms and Conditions

• Membership is NOT dental insurance but a payment arrangement offered by Smile Circle Dental for services rendered.
• Membership is offered exclusively to patients without dental insurance and cannot be combined with any dental insurance.
• Membership fee is due at enrollment, valid for 12 months from the date of enrollment, and is non-refundable.
• Treatment discount is applicable only when full payment is received on the day of treatment.
• Only applicable to services provided by a smile circle general dentist. Any specialty services that require a specialist are not covered.
• It is solely the patient's responsibility to schedule and keep their appointments. No refunds will be issued for any reasons including failure to schedule and maintain appointments.
